A British mother died from a broken toe after a large blood clot beneath her plaster cast broke free and became stuck on her lung.

Deborah Bowden's family is suing the UK's National Health Service for $2.7 million, claiming she would still be alive if doctors had booked a routine check-up on-time, the News of the World reports.

Mrs Bowden, 39, went to the Stepping Hill Hospital for an operation on her right big toe after breaking it when she slipped on stairs at the family's home in Greater Manchester on August 30 last year.

She was told hospital staff would contact her in a fortnight to check the stitches.

Mrs Bowden's husband Lee said when she hadn't heard anything for 12 days, she contacted the hospital and spent a week trying to make an appointment.

At the same time Mrs Bowden began having chest pains and, on September 16, her doctor diagnosed her with a chest infection.

The next day, a hospital staff member contacted Mrs Bowden to make a check-up appointment for September 22 — more than three weeks after the accident.

But Mr Bowden said his wife suddenly dropped dead on September 20, as she was travelling as a passenger in a car to work with her two children in the back.

"Deborah was my life. I cradled her in my arms as she slipped away ... killed by a broken toe, which is just ridiculous," he was quoted as saying.

"She just let out a groan and fell forward ... I stopped and got her on to the pavement ... I tried to revive her but I knew she'd gone."

Mr Bowden said he believed if hospital staff had made the booking on-time, the clot would have been found and his wife would still be alive.

The couple's two children, two-year-old Luke and Paige, 10, were still distraught after witnessing their mother's death and checked cupboards at home thinking she might be "hiding", he said.

Coroner John Pollard last week ruled Mrs Bowden's death as accidental but slammed the hospital's "complex" patient recall procedures as being set-up to fail.
